In our company,

BRD(Business Requirement Document) will be received from the clients.
It will be analized by PM and Project Plan(It includes Resources,WBS,Date of completion) will be made by him.From that project story line will enhanced and discussed with Project Lead.

Then SRS will be made by PL.From that taks will be split and given to the programmers.Mostly requirements will be communicated through mail.And that will be in word format.
